【Main ingredient】Kanamycin Sulfate

【Characters】This product is a colourless to pale yellow or pale yellow-green clear liquid.

【Pharmacological actions】

Pharmacodynamics Kanamycin is an aminoglycoside antibiotic with an antibacterial spectrum similar to that of streptomycin, but with a slightly stronger effect.  It has a powerful antibacterial effect on most gram-negative bacilli such as Escherichia coli, proteus species, Salmonella and Pasteurella multocida, and is also sensitive to Staphylococcus aureus and Mycobacterium tuberculosis.  Pseudomonas aeruginosa, gram-positive bacteria (except Staphylococcus aureus), rickettsiae, anaerobic bacteria and fungi are resistant to this product.

Similar to streptomycin, sensitive bacteria are susceptible to kanamycin resistance.  Cross-resistance with neomycin and unidirectional cross-resistance with streptomycin.  Acquired resistance is often seen in Escherichia coli and other gram-negative bacteria.

Pharmacokinetics It is rapidly absorbed by intramuscular injection and reaches peak blood concentration in 0.5-1.5 hours. It is widely distributed in pleural fluid, ascites and parenchymal organs, but rarely penetrates into saliva, bronchial secretions and normal cerebrospinal fluid.In meningitis, the concentration of the drug in the cerebrospinal fluid is approximately doubled. Concentrations in bile and faeces are very low. It is mainly excreted by glomerular filtration, 40%-80% of the injected dose is excreted in the urine in its original form. 2 hours after an intramuscular injection of 75mg/kg to goats, the concentration in the urine can reach 8300μg/ml. A small amount can be excreted in milk. The elimination half-life in horses, buffaloes, yellow cows, dairy goats and pigs after intramuscular injection is 2.1 to 2.8 hours.

【Drug interaction】

(1) Synergistic effects occur when used with penicillins or cephalosporins.

(2) The antibacterial effect is enhanced in an alkaline environment.  Combining with alkaline drugs   (e.g. sodium bicarbonate, aminophylline, etc.) can enhance the antibacterial efficacy, but the toxicity is also enhanced accordingly.  When the pH exceeds 8.4, the antibacterial effect is weakened.  

(3) Combination with cephalosporins, dextran, potent diuretics (e.g. furosemide, etc.) and erythromycin may enhance the ototoxicity of this product.  

(4) Skeletal muscle relaxants (e.g. succinylcholine chloride, etc.) or drugs with such effects may enhance the neuromuscular blocking effect of this class of drugs.

【Effect and use】

Aminoglycoside antibiotic.  Used for the treatment of septicaemia and infections of the urogenital tract and respiratory tracts, also for porcine asthma.

【Usage and dosage】

Intramuscular injection: a single dose of 0.1-0.15ml per 1kg body weight for domestic animals. 2 times a day ,3-5 days consecutive days.

【Untoward effect】

(1) Kanamycin has the same ototoxicity and nephrotoxicity as streptomycin, and its ototoxicity is stronger than that of streptomycin and gentamycin.

(2) Neuromuscular blocking effects are often caused by overdose.

【Cautions】

(1) Cross-sensitization with other aminoglycosides and contraindicated in animals with aminoglycoside hypersensitivity.

(2) Use with caution when the animal is dehydrated or has impaired renal function.

(3) When treating urinary tract infections, internal administration of sodium bicarbonate may enhance the efficacy of the drug.

(4) Ca2+, Mg2+, Na+, NH4+, K+ and other cations may inhibit the antibacterial activity of this product.

(5) In acute poisoning, anticholinesterase drugs such as neostigmine and calcium preparations (calcium gluconate) can be used to antagonize its muscle conduction blocking effect.
